Importance and Scope

Dramatic rises in obesity in children have been documented over the last few decades and this will have very important consequences for child and adult health. It is increasingly recognised that the pathway to obesity starts in the first years of life, even before children reach school age. Obesity is known to be caused by excess intake of calories and lack of physical activity, but many other putative factors have been suggested to play a role. New hypotheses center on vulnerable periods during pregnancy and in the first years of life and include maternal weight gain during pregnancy, maternal smoking, and exposure to environmental pollutants, breastfeeding, and infant sleep duration. In addition, factors in the social and built environment are increasingly examined for their role in the childhood obesity epidemic. Pregnancy and early life have thus far received little attention in obesity prevention campaigns and interventions in children focusing exclusively on individual modification of the energy balance increased energy expenditure or decreased caloric intake have been of limited success. Amongst new risk factors proposed for childhood obesity are environmental chemical pollutants. The aim of this project is to evaluate the influence of prenatal and early life environmental and social factors on the development of childhood obesity up to age 4 years in Barcelona.

 

As with the adult data, the country uses the international cut-off point for children reaching the recommended physical activity levels, endorsed in WHO are Global Recommendations on Physical Activity for Health 2010. The ALADINO Study on food, physical activity, child development and obesity, which forms part of the Strategy for Nutrition, Physical Activity and the Prevention of Obesity NAOS Strategy, which in turn forms part of the WHO Regional Office for Europe’s Childhood Obesity Surveillance Initiative COSI, collects information on nutrition, physical activity and lifestyles in children aged 6—9 years. The latest Health Behaviour in School-aged Children study reports data disaggregated by age. Unpublished data for Spain from the 2013/2014 round of the study are reported and showing that only 24.4% of adolescents reach the recommended physical activity levels for health according to the WHO recommendations, with significantly more boys being active than girls 31.7% and 17.3%, respectively.

 

Why Barcelona:

“Fifteen years ago, nine percent of infants and adolescents were obese,” said a physician who heads Childhood Digestive. “Now 18 percent are obese and around 14 percent are overweight.”

The secret to success was a surgical technique known as vertical banded gastroplasty, in which the volume of his stomach was permanently reduced by stapling a portion of it shut, immediately diminishing the quantities he consumed.

There is another statistic in an explosion of childhood and adolescent obesity that has spread not just globally, but especially in Spain. The latest studies completed for Spain by AVENA and Enkid, report that one in four Spanish children and adolescents are overweight or obese, with Barcelona corresponding to the national average.

Obesity is defined by the World Health Organisation WHO as a medical condition in which excess body fat has accumulated to the extent that it may have an adverse effect on health. Its prevalence in youth can have a long-term impact: of the children who are obese at 16, 90 percent become obese adults, which can contribute to arteriosclerosis, obesity, cancer, hypertension, osteoarthritis, gout and various heart related diseases.

 

The causes of obesity among young people are of various types, we explained to Metropolitan. “Fundamentally, there’s a genetic factor. Then there are endocrine diseases and digestive diseases, although diseases constitute a small percentage of what causes obesity.”

The primary cause for the boom in obesity, he said, comes from changing personal habits. “A human being is genetically prepared to conserve all the calories that he can, because we’ve always eaten little and had to work a lot. In the past 50 years, this has become inverted in the first world. Now, there is a lot of food and very little movement.

Economic factors can also contribute to childhood obesity, because low-income parents must work longer hours and therefore lack the time and resources to buy and prepare fresh and healthy food. What’s more, wealthier parents are able to pay for extra-curricular activities like sports and dance, where children are able to burn away calories. One Spanish study, published in 2009, found a direct correlation between income and obesity. Communities with a per capita income of €24,100 suffered a 14.9 percent overweight and obesity rate in their children, while a per capita income of €8,800 corresponded to 29.4 percent.

The worldwide rise in overweight and obesity in childhood and adolescence in recent years has been dramatic. It is estimated that around one third of all children are overweight. In Spain, the prevalence of child obesity is among the highest in Europe, together with Malta, Italy, Greece, and the United Kingdom. In 2010 the SESPAS Spanish Society of Public Health and Health Administration reported that 35% of children had excess weight 20% were overweight and 15% were obese, making this phenomenon an increasingly important and highly significant risk factor, whose trend should be rigorously and systematically monitored. Methodological weaknesses related to the definition and measurement of obesity and sample selection are, however, common

When the WHO criteria were used, the prevalence of overweight was 29.0% 96%CI, 26.2%-30.5% for the Spanish population aged 8–17 years; the prevalence of obesity was 17.2% 97%CI, 14.3%-19.2%, and total excess weight overweight plus obesity was 41.7% 97.5%CI, 39.5%-45.4%. Problems with obesity were more common in the group aged 8-13 years than in those aged 14-17: 34.3% were overweight and 18.5% were obese in the first group compared to 20.2% and 10.7%, respectively, in the second group P<.01.By sex, 31.5% of Spanish boys were overweight and 15.2% were obese compared with 25.7% and 15.5% of Spanish girls, respectively. No statistically significant differences were observed between childhood and adolescent overweight and obesity in either sex according to municipality size. The educational level of the person responsible for the child's diet mother in 85% of subjects; father in 14% was statistically associated with the prevalence of obesity or overweight. In particular, the highest percentage of child obesity was seen when the person responsible for the children's diet and care had only finished 8th grade or the first 2 years of vocational training.

The WHO GHO 2010 estimates for the Spanish adolescent population defined as aged 11—17 years in relation 0074o the WHO data 6, reveal slightly lower figures for adolescents reaching the recommended physical activity levels 22.8%; 30.3% for boys and 15.7% for girls than those from the 2013/2014 round of the HBSC study

In 2013, 42 million infants and young children were overweight or obese, worldwide and 70 million young children will be overweight or obese by 2025 if current trends continue. Without intervention, overweight infants and young children will likely continue to be overweight during childhood, adolescence and adulthood. Obesity in childhood is associated with a wide range of serious health complications and an increased risk of premature onset of illnesses, including diabetes and heart disease.
